Chasing a moving target: outcome and risk stratification in patients with transposition of the great arteries after atrial switch operation.
نویسنده
چکیده
Before the advent of the atrial switch operation, hardly any infant born with complete transposition of the great arteries (TGA) survived beyond the first year of life. In the 1950s and 1960s, two operations introduced by Senning and Mustard, respectively, revolutionized the outcome of the condition, allowing patients to survive well into adulthood. However, ongoing long-term complications, as well as increased morbidity and mortality remain an issue even five decades later. This type of physiological repair has now been largely replaced by the arterial switch procedure. Today it continues to be reserved only for highly selected cases and has, thus, become part of medical history more than being a viable treatment option.

Background: Balloon atrial septostomy is an emergent procedure in pediatric cardiology. Nowadays, most patients in need of the procedure have acceptable outcomes after surgical repair. Thus, it is important to perform this procedure as safe as possible. By performing early arterial switch operation and prostaglandin infusion, the rate of balloon atrial septostomy has markedly decreased. Howeve...
متن کاملPatients after atrial switch operation for transposition of the great arteries can not increase stroke volume under dobutamine stress as opposed to patients with congenitally corrected transposition.
BACKGROUND Patients after atrial switch operation for transposition of the great arteries have limited exercise performance. Rigid atrial baffles may cause a relative preload reduction. Previous studies have had suboptimal control groups, which ideally should consist of patients with congenitally corrected transposition of the great arteries (ccTGA) without previous heart surgery, having a syst...

Background: Nowadays, the treatment of choice for anatomical correction of transposition of great arteries is arterial switch but some children are not good candidates for this operation. In these cases atrial switch or Senning procedure is an accepted method, thus outcome of this procedure needs to be better delineated. Methods: This prospective study included 65 patients that underwent Senni...
متن کاملTransposition of great arteries is associated with increased carotid artery stiffness.
Transposition of great arteries is the consequence of abnormal aorticopulmonary septation. Animal embryonic data indicate that septation and elastogenesis are related events, but human and clinical data are not available. We tested the hypothesis that large artery elastic function was impaired in patients with transposition of great arteries. We studied 34 patients aged 9 to 19 years, 12+/-3 ye...
متن کاملHeart transplantation in atrial switch and in congenitally corrected transposition of the great arteries
Background Patients with congenitally corrected transposition of the great arteries (ccTGA) and those who underwent atrial switch eventually face systemic ventricular failure due to deterioration of the morphological and structural function. Heart transplantation is at present one of the two best available options. We report our institutional experience of heart transplantation in this group, w...
